Personal health and medical information is collected directly from residents, their relatives and other authorised personnel such as General Practitioners, NASC, Hospitals, other referrers and through our own observations and assessments.
Personal health and medical information is used and disclosed solely for the purpose of providing quality aged care to the residents and/or to further the health and safety of the other residents within our facility.
Use of your Personal Information
We collect and use your personal information to deliver the services you have requested.
We do not use or disclose sensitive personal information, without your explicit consent.
We will disclose your personal information, without notice, only if required to do so by law or in the good faith belief that such action is necessary to:
- Comply with legal process served on us
- Protect and defend the rights or property of our facility; and,
- To protect the personal safety of users of our facility, or the public.

We secure your personal information from unauthorised access, use or disclosure.
You can expect that your health information
- Is kept confidential, as it is collected in a situation of confidence and trust.
- Is treated as sensitive, because it may include details about body, lifestyle, emotions and behaviour.
- May have ongoing use if a piece of medical information becomes clinically relevant even a long time after it was initially collected.
- Will be used for the purposes for which it was originally collected and you will be told about those purposes.
